Join a pediatric network built for providers
ChildNet supports physicians and practices with contracting, credentialing, health-plan coordination, and a shared network of pediatric expertise.
Contracted Network Providers
ChildNet’s Network Participation Agreement (NPA) gives members access to participating health plans and payors, along with support services such as group purchasing.
To request membership as a Network Provider, complete the Network Provider Request Form and return it to ChildNet. Your request will be reviewed by the ChildNet Board of Directors.
Eligibility
Before requesting an application, please review the core eligibility requirements for contracted Network Providers.
Medical Services
Provide medical services as a Pediatric/Family Practice Primary Care Physician or Pediatric Specialist.
Licensing
Hold a current, valid California medical license. Applicants must certify that their license has never been revoked or suspended by any state licensing board.
Privileges
Hold, or be applying for, medical staff appointment with Valley Children’s Hospital or another approved facility with privileges appropriate to the services provided.
Certification
Be board certified or have completed a residency-training program approved by the ACGME, AOA, or another accrediting body acceptable to CNMA’s Board in your current specialty.
If you meet the qualifications, complete the Network Provider Request Form and return it to ChildNet. If approved by the Board of Directors, you will receive a full application packet.
A $200 credentialing fee is charged initially and bi-annually for recredentialing.
After your credentialing application is complete and approved, ChildNet will send a Network Participation Agreement. Once signed, accepted health plan agreements will be notified to add you to the ChildNet panel.
