ChildNet Medical Associates provides its physicians and members a network of shared knowledge and experiences, contracting facilitation, credentialing with health plans, and payor/provider network support.
ChildNet’s Network Participation Agreement (NPA) provides members with access to a variety of health plans and payors. In addition, providers have access to value-added services such as a Group Purchasing Program. The NPA covers both individual and group practice arrangements; exhibits attached to the NPA define these relationships.
To request membership as a Network Provider in ChildNet, please complete the Network Provider Request Form and return it to ChildNet. Your request will be presented to the ChildNet Board of Directors for approval.
Before you request an application for membership as a contracted Network Provider in ChildNet, please ensure that you meet the following eligibility.
Medical Services
Must provide medical services as a Pediatric/Family Practice Primary Care Physician or Pediatric Specialist.
Licensing
Must possess a current, valid license to practice in the State of California and certify that your license to practice has never been revoked or suspended by any State licensing board.
Privileges
Must currently possess Medical Staff appointment with Valley Children’s Hospital or another approved facility with privileges commensurate with the services to be performed as a Contracted Physician/Provider, or will be applying for staff appointment.
Certification
Must be board certified or completed a residency-training program approved by the Accreditation Council for Graduate Medical Education (ACGME), American Osteopathic Association (AOA), or other accrediting body acceptable to CNMA’s Board in current specialty.
If you meet the listed qualifications and would like to become a Network Provider with ChildNet, please complete the Network Provider Request Form and return to ChildNet. Upon approval of your request by the ChildNet Board of Directors, an Application will be sent to you. Please return the Application and all required information as soon as possible.
You will be charged a credentialing fee of $200.00 initially and bi-annually for recredentialing.
Upon completion and Board approval of your credentialing application, a Network Participation Agreement will be sent to you. Once ChildNet has received your completed and signed Network Participation Agreement, any Health Plan agreements that you accept participation in will be notified to add you to the ChildNet panel.
